- Proof of address of the student and relative applying behalf of student
- Proof of occupation of the parents
- Copies of all prior educational qualifications
- Copy of birth certificate or school leaving certificate
- Copy of latest Bank Statement or updated passbook of parents
- In case the applicant proposes to study in a foreign country, mention reasons as why admission in foreign university is considered essential.
(Applicant should produce a certificate from a University professor or Principal of a College of a well-known educationalist in this regard)

- Doctor’s or Hospital Statement in original with details about the estimated cost of treatment
- Hospital bills and Medical bills (not more than six months old) for expenses incurred in original
- Recommendation letter of medical social worker/Hospital
- Copy of income certificate of the patient and relative applying on behalf of the patient
- Copy of PAN number of the patient or relative applying on behalf of the patient
- Copy of latest Bank statement or updated pass book of patient or relative applying on behalf of the patient
